PLYMOUTH â Michael T. Haywood, 45, of Plymouth died Thursday, June 6, 2013 at the St. Joseph Regional Medical Center in Plymouth.
He is survived by his mother, Sallie Haywood of Bremen; a brother, Jim (Nancy) Haywood of Elkhart; and a sister, Sue Haywood of Lexington, Ky.
A funeral service will be held at 11 a.m. Tuesday, June 11, 2013 at the Mishler Funeral Home, Bremen. Burial will follow in the Bremen Cemetery. Visitation is from 4-8 p.m. Monday, June 10, 2013 at the funeral home.

WALKERTON â Helen M. Lazarek, 96, died Tuesday, June 4, 2013 at IU Health LaPorte Hospital, LaPorte.
Survivors include one daughter, Kathryn (Peter) Luther of LaPorte; two sons, James Foster of Mansfield, Ohio and Thomas (Carolyn) Foster of Walkerton; six grandchildren; 11 great-grandchildren and one brother, John Bellinger of Florida.

PLYMOUTH â Sam Le Roi Forsythe, 45, passed away Tuesday, June 4, 2013 after a four year courageous battle with cancer.
Sam was born March 24, 1968 in Plymouth to Sam L. Ogle and Phyllis (Mishler) Ogle and he was later adopted by Vern L. Forsythe. On Oct. 11, 1994, he married Lori (Elder) Nieubuurt. Their union was blessed with four daughters; Catherine Elizabeth, Sami Jo, Julieann and Eve Marie. Lori brought to the marriage two sons and one daughter; Brad Martin (Tania) Nieubuurt, Shane Andrew Nieubuurt and April Elaine (Josh) Martin.

BREMEN â Almeda âJoâ Josephine Hendrix Edwards, 92, of Bremen passed away Monday June 3, 2013 at her daughtersâ home, in Hospice Care.
She was born at home Dec. 9, 1920 in Ashland, Neb., the daughter of Daniel and Ethel Clouse Hendrix. Her siblings Gladys, Grace, Norma, Frances, Lotus, Virginia, and Daniel all preceded her in death. Also preceding her in death was a son Mark Hunter, and four sons-in-law.

PLYMOUTH â Randall O. Kreier, 54, a Plymouth resident for the last 20 years, coming from Streator, Ill., passed away at 9:30 p.m. Monday, June 3, 2013 at his residence under the loving care of his family and hospice.
The son of Ronald and Margaret Evans Kreier, Randy was born July 5, 1958 in St. Louis, Mo. He attended Woodland High School.

PLYMOUTH â Juanita H. Marquez, 82, of Plymouth, passed away Monday, June 3, 2013, at 10:32 a.m., at Saint Joseph Regional Medical Center, in Plymouth.
Juanita was born Aug. 29, 1930, in Santa Rosa, Texas, to Marcos and Esther (Olivarez) Hernandez. Juanita and Manuel Marquez were married Feb. 19, 1950, in Santa Rosa, Texas. They moved to Plymouth in 1967. Manuel passed away June 18, 2004. Juanita worked at the former Parkview Hospital, in Plymouth, for many years.
She was a member of Saint Michael Catholic Church.

FRANKFORT â Cassandra Nicole âCaseyâ Brown, 24, of Frankfort, formerly of Tippecanoe County, passed away as a result of injuries suffered in an automobile accident Sunday, June 2 in Clinton County. Born Nov. 4, 1988 in Panama, she was the daughter of Bryan L. Brown (wife, Ellen D.) of Walkerton and Michelle D. Stembaugh (husband, Scott A.) of Dayton. She attended McCutcheon High School.Â
Cassandra was the assistant manager of Super Pantry in Lafayette. She enjoyed listening to music and spending time with her baby.

NAPPANEE â Doris Ann Heckaman, 71, of Nappanee, died Monday, June 3, at 1:15 p.m. at her residence from natural causes.
Surviving are daughters, Sharie Heckaman and Lisa (Brian) Lewis, both of Nappanee; son, Brian Heckaman of Nappanee; sisters, Lynn Whittenberger of Kokomo, Janice (James) Fackler of Syracuse, and Margaret Bell of Matteson, Ill.; and brothers, William Berger of New Paris and Steven (Connie) Berger of Wallingford, Vt.
